Have you served in the armed forces?

You are a veteran if you have served in the armed forces for at least 1 day.

During and after your discharge period, there are a number of actions you can take to help NHS and specialist veteran services support you.

Indicating you are a veteran in your NHS records will make sure you are given access to services dedicated to those that have served in the armed forces. If you are a patient at one of the practices in our Primary Care Network you can inform us of your veteran status by filling in the form below, alternatively you can visit your practice and speak to the receptionist.
